29 CFR 1926.651(k)(1): An inspection of the excavation, the adjacent areas, and protective systems was not conducted by a competent person prior to the start of work and as needed throughout the shift: The employer failed to conduct an inspection of a trench 8 feet in depth, 2 feet wide at cut depth, 5 feet wide at grade, 28 feet in length, and comprised of material consistent with that of a Type B soil. The trench lacked cave-in protection and the spoil pile was improperly placed. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
