CFR 1926.502(b)(1):  The top edge height of top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level;  (a) B&H General Contractors, 450 West Main Street, New Castle, CO 81647:  On and before July 1, 2011, six employees working on a concrete slab and a partially covered construction area located at 450 West Main Street, New Castle, CO were not protected from falling by a system as described above.  Specifically, employees were exposed to fall hazards from an improperly constructed and partially erected guardrail system which did not meet the requirement to be at 42 inches (plus or minus 3 inches) above the walking working level. Condition exposed employee to fall hazards of over 10 feet into the open basement section of the New Castle Public library renovation at the area around the interior stairwell.  (b)B&H General Contractors, 450 West Main Street, New Castle, CO 81647:  On and before July 1, 2011, six employees working on a concrete slab and a partially covered construction area located at 450 West Main Street, New Castle, CO were not protected from falling by a system as described above. CFR 1926.502(b)(2):  Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members were not installed between the top edge of the guardrail system and the walking/working surface where there was no wall or parapet wall at least 21 inches (53 cm) high;  (a)B&H General Contractors, 450 West Main Street, New Castle, CO 81647:  On and before July 1, 2011, six employees working on a concrete slab and a partially covered construction area located at 450 West Main Street, New Castle, CO were not protected from falling by a system as described above.  Specifically, employees were exposed to fall hazards due to the lack of mid-rails at a guard rail system without a parapet or some other protection put into place. Condition exposed employee to fall hazards of over 10 feet into the open basement section of the New Castle Public library renovation. Specifically, employees were exposed to fall hazards due to the lack of mid-rails at a guard rail system without a parapet or some other protection put into place. Condition exposed employee to fall hazards of over 10 feet as they worked around an open area looking down into the basement area through the interior starewell to the basement. CFR 1926.1051(a): A stairway or ladder was not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided  (a)B&H General Contractors, 450 West Main Street, New Castle, CO 81647:  On and before July 1, 2011, six employees working on a concrete slab and a partially covered construction area located at 450 West Main Street, New Castle, CO were not provided a stairway or ladder to access the main working slab.  Distance from floor to main slab was approximately 22 inches, and varied due to changes in elevation at the various points used as access.
