29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) An employee was exposed to fall hazards of approximately 15 feet while working from a scaffold. The working level was not fully planked. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

An employee was exposed to fall hazards of approximately 15 feet while working from a scaffold. The scaffold was not erected under the supervision of a competent person.
