29 CFR 1910.178(l)(6):   Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) At the Lincoln Manufacturing facility located in Strasburg, Ohio, the employer was unable to produce documentation that certified the forklift training of the forklift operators that had been administered by the Buckeye Career Center.

29 CFR 1910.178(p)(1):     If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.    a) At the Lincoln Manufacturing facility located in Strasburg, Ohio, there was a Toyota forklift (Model 7FGU25, serial # 79880) that had a horn that did not work and was not removed from service.

One or more methods of machine guarding shall be provided to protect the operatorand other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips andsparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.\r\n\r\na) At the Lincoln Manufacturing facility, in Strasburg, Ohio, the employer had a AMADA 400W horizontal band saw in the saw room that had an unsed portion of the blade that was not guarded.

29 CFR 1910.305(g)(1)(iv)(A):  Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used: As a substitute for the fixed wiring of a structure;  a) At the Lincoln Manufacturing facility, located in Strasburg, Ohio, the employer had an extension cord that was used as a substitute for permanent wiring of an outdoor flood light.  The extension cord ran above the overhead door by the maintenance shop, and had the female end removed, so that it could be directly connected to the flood light above the overhead door.

29 CFR 1910.305(g)(1)(iv)(B): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  Where run through holes in walls, ceilings, or floors;  a) At the Lincoln Manufacturing facility located in Strasburg, Ohio the employer had a flexible cord used to power an external flood light over an overhead door by the maintenance shop that had been run through the wall to get power from the receptacle inside the building to the light outside.
