29 CFR 1910.23(a)(1): Every stairway floor opening shall be guarded by a standard railing constructed in accordance with paragraph (e) of this section. The railing shall be provided on all exposed sides (except at entrance to stairway). For infrequently used stairways where traffic across the opening prevents the use of fixed standard railing (as when located in aisle spaces, etc.), the guard shall consist of a hinged floor opening cover of standard strength and construction and removable standard railings on all exposed sides (except at entrance to stairway).    On the second floor of the chicken barns, employees were using walkways to access chicken cages and were exposed to unguarded manure chute openings.  The chute openings were adjacent to walkways and measured approximately 5 feet by 17 inches.  Employees were exposed to a fall hazard of approximately 10 feet above the concrete floor.

29 CFR 1910.333(a): "General." Safety-related work practices shall be employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work is performed near or on equipment or circuits which are or may be energized. The specific safety-related work practices shall be consistent with the nature and extent of the associated electrical hazards.      On or about June 15, 2011, employee(s) were exposed to electrical shock and are flash hazards while performing a 208 volt motor replacement for a bank of ventilation fans in the barns.  Safe work practices and electrical protective equipment were not utilized during this work.
