Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm from impact and struck by hazards, in that, two anchors did not have inspection tags on them of when they were tested and two anchor tags were out of date:\\\\r\\\\n\\\\r\\\\na) On or about June 22, 2011, and at times prior, employees were exposed to injury in that two anchor tags did not have tags on them of when they were tested and two anchor tags were out of date.\\\\r\\\\n\\\\r\\\\nAmong other methods, one feasible and acceptable method to correct this hazard is to inspect all anchor tags in accordance with RP4G, 2004 Section 14.5. \\\\r\\\\n\\\\r\\\\nAbatement Note: Abatement certification and documentation is required for this item (see enclosed \\\\\\\"Sample Abatement-Certification Letter\\\\\\\").\\\\r\\\\n
