29 CFR 1910.151(c):   the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  (a) Larchwood Inns at 2845 North 15th St. in Grand Junction, CO: On and before June 30, 2011 the facilities for quick drenching or flushing of the in B Hall Janitorial #2 was not functional in that the water to sink where the eyewash was mounted was turned off.  This condition exposed employees using corrosive cleaning chemicals to an eye hazard.    Note: Abatement certification is not required for this item.

29 CFR 1910.1030(c)(1)(iv): \\\\r\\\\n\\\\r\\\\nThe Exposure Control Plan was not reviewed and updated at least annually and whenever necessary to reflect new or modified tasks and procedures which affect occupational exposure and to reflect new or revised employee positions with occupational exposure:\\\\r\\\\n\\\\r\\\\n(a) Larchwood Inns at 2845 North 15th St. in Grand Junction, CO: The employer did not review and update the Exposure Control Plan at least annually.  The Plan provided during the inspection was dated March 2009.  The employer did not document annual consideration and implementation of safer medical devices.  The employer did not solicit non-managerial employees\\\\\\\' input to identify, evaluate, and select effective engineering and work practice controls. This condition exposed employees to sharps injuries. \\\\r\\\\n\\\\r\\\\nAbatement Note: The review and update of such plans shall:\\\\r\\\\n- Reflect changes in technology that eliminate or reduce exposure to bloodborne pathogens; and\\\\r\\\\n- Document annually consideration and implementation of appropriate commercially available and effective safer medical devices designed to eliminate or minimize occupational exposure.\\\\r\\\\n- An employer, who is required to establish an Exposure Control Plan shall solicit input from non-managerial employees responsible for direct patient care who are potentially exposed to injuries from contaminated sharps in the identification, evaluation, and selection of effective engineering and work practice controls and shall document the solicitation in the Exposure Control Plan.  Abatement Note: The standard does not specify the level of detail that must be included in this documentation; however, sufficient information must be provided to substantiate the facility's judgment. As discussed in the preamble of the Final Rule, consideration and implementation of safer medical devices could be documented in the Exposure Control Plan by describing the safer devices identified as candidates for adoption; the method or methods used to evaluate devices and the results of the evaluations; and justification for selection decisions. 29 CFR 1910.1030(h)(5)(i): \\\\r\\\\n\\\\r\\\\nThe employer did not establish and maintain a sharps injury log for the recording of percutaneous injuries from contaminated sharps. The information in the sharps injury log shall be recorded and maintained in such manner as to protect the confidentiality of the injured employee. \\\\r\\\\n\\\\r\\\\n(a) Larchwood Inns at 2845 North 15th St. in Grand Junction, CO: The employer did not establish and maintain sharps injury logs for recording percutaneous injuries form contaminated sharps.  There were at least 4 sharps injuries in the past four years.  This condition prevented the employer from identifying trends associated with sharps injuries.\\\\r\\\\n\\\\r\\\\nAbatement Note: The sharps injury log shall contain, at a minimum:\\\\r\\\\n- The type and brand of device involved in the incident,\\\\r\\\\n- The department or work area where the exposure incident occurred, and\\\\r\\\\n- An explanation of how the incident occurred.
