29 CFR 1910.110(f)(2)(i): Containers in storage shall be located so as to minimize exposure to excessive temperature rise, physical damage, or tampering by unauthorized persons. Shipping area - On or about July 6, 2011, in the shipping/receiving area twelve (12) L.P. gas containers were located and exposed to physical damage by forklift truck travel. Employees were thereby exposed to the hazards associated with damaged gas cylinders.

29 CFR 1910.334(a)(2)(ii): If there is a defect or evidence of damage that might expose an employee to injury, the defective or damaged item shall be removed from service, and no employee may use it until repairs and tests necessary to render the equipment safe have been made. Shop area - On or about July 6, 2011, an extension cord used to power a table saw was hazardous in that the insulation was damaged and the conductor were exposed. Employees were thereby exposed to electric shock or electrocution hazards.

29 CFR 1910.157(g)(1): Where the employer has provided portable fire extinguishers for employee use in the workplace, the employer shall also provide an educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting.
