Section 5(a)(1) of the Occupational Safety and Health Act of 1970; The employer did not furnish employment and a place of employment which was free from recognized hazards which were likely to cause death or serious physical harm to employees in that, two of the anchors did not have inspection tags on them of when they were tested and the other two were out of date:\\r\\n\\r\\n\r\n\r\na) On or about June 22, 2011, and at times prior,  employees were exposed to while working on a workover rig in that, two the anchors did not have inspection tags of when they were inspected and the other two anchor tags were out of date..\\r\\n\\r\\n\r\n\r\nAmong other methods, a feasible and acceptable method to correct this hazard is to inspect all anchors tags in accordance with the American Petroleum Institute (API) Standard RP4G 2004, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations.\\r\\n\\r\\n\r\n\r\nAbatement Note:  Abatement certification and documentation is required for this item (see enclosed \\\"Sample Abatement-Certification Letter\\\").\\r\\n
