Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    a. On or about June 30, 2011, at the above addressed jobsite, employees conducting roofing operations on a steep roof (6:12 pitch), twenty three (23) feet above the ground, were not protected from falling, thereby exposing the employees to the hazards associated with falls.

On or about June 30, 2011, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3) feet above the upper landing surface.  Employees were thereby exposed to the hazards associated with falls.
