29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition: (a) On or before June 29, 2011, the host employer did not ensure the control of ignition sources while employees of a subcontractor power washed the inside of storage tanks containing flammable vapors. On June 29th, 2011, two employees were seriously injured after the atmosphere inside the 300 barrel storage tank exploded.

29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardwareshall were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources. a) Occidental Petroleum, East Plateau Site @ My Way Ranch 17-2, Collbran, CO: Prior to pressure washing the inside of storage tanks, the employer (host) did not provide circuit breaker locks capable of de-energizing and ioslating the 6 internal storage tank heaters. On or about June 29, 2011. two subcontractor employees power washing the inside of a 300 barrel storage tank were seriously injuried after the hazadous atmosphere ignited.

The hazardous sources of energy include but are not limited to the internal electric tank heaters and 4 inch ventilation lines interconnecting the 6 production/storage tanks. On or about June 29, 2011, two subcontractor employees power washing the inside of a 300 barrel storage tank were seriously injured after the hazardous atmosphere ignited.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: (a) Occidental Petroleum, East Plateau Site @ My Way Ranch 17-2, Collbran, CO: On or before June 29, 2011, the host employer had not ensured that adequate training was received by authorized employees to recognize and isolate all potentially hazards energy sources prior to tank cleaning. On June 28th and 29th 2011, authorized employees did not isolate and render inoperative the 6 internal tank heaters or the 4 inch ventilation lines interconnecting the 6 production/storage tanks. Two employees received serious injuries while power washing the interior of a 300 barrel condensate tank after the hazardous atmosphere ignited.
