29 CFR 1910.22(b)(1):   Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage were made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard.    Indoor scrap sorting floor.  Various scrap materials were piled on the floor in areas where the forklift truck and skid steer loader were used.  There were no clear and designated aisles and passageways for powered equipment and pedestrians.  The forklift truck came to close to the customer service desk.    There is no abatement documentation or certification required for this item.

29 CFR 1910.110(f)(6)(ii): Containers of liquified petroleum gas were not in a suitable enclosure or otherwise protected against tampering.    Outside Storage Area.  Propane tanks were stored outside of a protective storage cage.  Scrap materials such as old pallets were piled on top of the tanks, subjecting them to damage. 29 CFR 1910.132(d)(1):   The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment (PPE).     Entire Facility.  The employer did not conduct and certify a personal protective equipment (PPE) hazard assessment to determine the types of PPE employees should be required to wear when performing various tasks, such as steel-toed safety shoes to protect against metal scrap injuries, gloves to protect against battery acid and metal punctures and hard hats to protect against overhead or side impact hazards during the scrap sorting process. The employer did not ensure that each affected employee used protective footwear when working in areas where there was a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, and where such employee's feet were exposed to electrical hazards.    Entire Facility.   The employer did not require employees who worked in the inside scrap sorting area and who walked on top of scrap in outside trailers to wear steel-toed safety shoes to protect their feet against falling objects, and punctures from metal scrap. Sort and Scrap Room, Desk Area.  Relocatable power taps (electrical outlet strips) were used improperly in an industrial setting in the following instances:    a.  A relocatable power tap was tied onto a pipe in a vertical position and used to power a radio and time clock.    b.  A relocatable power tap was plugged into an extension cord and stretched in the air while used to power an       industrial fan. 29 CFR 1910.303(g)(1)(i)(C):  Electric Equipment Operating at 600 volts, nominal, or less to ground.  The work space was not clear and did not extend from the grade, floor, or platform to the height required by paragraph (g)(1)(vi) of this section.      Sort and scrap room.  A work table was placed in front and below the electrical cabinet that contained the overhead lighting circuit breakers, which restricted routine and emergency access to the breakers.
