Electrical wiring and equipment not subject to deposits of combustible residues but located in a spraying area as herein defined shall be of explosion-proof type approved for Class I, group D locations and shall otherwise conform to the provisions of subpart S of this part, for Class I, Division 1, Hazardous Locations: a. On or about July 1, 2011, in the Quonset hut, employees were required to perform spray finishing operations with flammable paint. The electrical outlets and wiring in the Quonset hut were ordinary and were not approved for use in a flammable spray finish area thereby exposing employees to fire and explosion hazards.

Electric lamps outside of, but within twenty (20) feet of any spraying area, and not separated there from by a partition, shall be totally enclosed to prevent the falling of hot particles and shall be protected from mechanical injury by suitable guards or by location: a. On or about July 1, 2011, in the Quonset hut, employees were require to perform spray finishing operations with flammable paint. The electrical lamps used with florescent bulbs were not enclosed to prevent the falling of hot particles thereby exposing employees to fire and explosion hazards.

Sorbent beds and filters shall be maintained and replaced or refurbished periodically following the manufacturer's instructions: a. On or about July 1, 2011, employees used the facility air compressor to supply breathing air to respirators worn by employees. The air supplied by the facility air compressor was not equipped with sorbent filters in-line and did not prevent employee exposure to unknown contaminants.
