29 CFR 1910.176(b):  Material storage system(s) were not secured to prevent tipping or collapse:       a) In the Shipping and receiving, H-D Disc, AM-Warehouse, and VIP-Aft H/T Department areas the pallet rack and material racks systems that were loaded and unloaded by fork trucks were not secured to the floor or braced by equivalent means to protect against accidental tipping and or total collapse.

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by ingoing nip points, rotating parts, and caught in moving parts:      Instance a); The small Grizzly Lathe located in the Die Set Maintenance Area did not have a chuck guard to protect against contact with the rotating chucks and employees utilized the lathe to debar and polish small die set pins.     Instance b): The perimeter guarding installed and located on the left hand side of the AIDA Press located in the Press Area did not have a working interlock switch to shut down rotating and conveying feed equipment to the AIDA Press when the gate was opened and the operator and helper periodically had to enter that gate to access and correct miss fed and or jammed parts.    Instance c): Located on the right hand side of the AIDA Press located in the Press Area did not have perimeter guarding or equivalent shielding from inadvertent contact with the part handling and reciprocating equipment where the operator enter along side the machine to make adjustment to the pick-up conveyor device.    Instance d): The parts handling equipment partially enclosed on both the K - Leveler and M- Leveler automotive controlled machines located in the Press Area did not have a guard installed to protect inadvertent contact to the pinch point created as the material handling ram equipment moved out side the machine and stop against an end bracket during machine automotive cycle.

29 CFR 1910.219(f)(3):  Sprockets and chains. All sprocket wheels and chains shall be enclosed unless they are more thanseven (7) feet above the floor or platform.     Instance a)  The sprocket wheel and chain powered conveyor located approximately 20 inches above the floor at the end of the Leveler parts conveyor line where the employees off loaded parts as received was not enclosed to protect against inadvertent contact and the ingoing nip points created by that equipment.     Instance b) Employees who loaded parts on the turn table power by sprocket wheel and chain located at the Heat Treat machine were exposed inadvertent contact to the ingoing nip points created by that equipment.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.    Instance a)  The acetylene and oxygen gas cylinders were stored together on a wheel cart and signally against the wall located in the maintenance parts room and were not secured to prevent being knocked over.

29 CFR 1910.253(b)(2)(iv):     Valve protection caps, where cylinder is designed to accept a cap, shall always be in place, hand-tight, except when cylinders are in use or connected for use.    Instance a)  Located in the maintenance parts room both the Acetylene and Oxygen gas cylinders that were not in use, which were design for valve protective caps, did not have protective caps installed on them to protect the valves from damage.
