29 CFR 1926.502(f)(1)(iii):     Points of access, materials handling areas, storage areas, and hoisting areas were not connected to the work area by an access path formed by two warning lines:    a)  Employees who accessed a roof edge to enter a work area walked from the roof edge to cross under a warning line without any designated path marked with warning lines between the roof edge and warning line system.    b)  Employees who were removing roofing materials from the forks of a rough terrain lift truck were walking between the roof edge and the warning line system without any designated path to connect to the work area.

29 CFR 1926.502(f)(2)(ii):     The rope, wire, or chain was not rigged and supported in such a way that its lowest point (including sag) was no less than 34 inches (.9 m) from the walking/working surface and its highest point is no more than 39 inches (1.0 m) from the walking/working surface;    Employees who were working on a roof were utilizing a warning line system which was constructed at a height greater than 39 inches above the walking/working surface.
