29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points: (a) For the employee who suffered fatal injuries resulting from contact with a metal grate that had penetrated through the insulation of a 480-volt power cord that was running along the ground, located at Ross 5603-42-10H well site near Williston, North Dakota. Note: This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 310593645, Citation 1, Item 6, and was affirmed as a final order on January 8, 2008, with respect to a workplace located at Andy's Mesa 69, Naturita, Colorado.
