Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: The employer did not maintain the condition of the flexible, high pressure hydraulic spinner hose, Spin Master's Model #550-H, for the rig floor. The hose was pinched against the spinner housing. The soft rubber cover was split open and bent exposing the hard hose line. This situation could lead to the failure of the hose due to lack of maintenance. This condition exposes employees to struck-by hazard.    Abatement Note: API 7K and 7L certification requires proper maintenance of the vibrating hoses. By not guarding the rotating parts on the water pump, employees were exposed to laceration hazards.    (b) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not provide one or more methods of machine guarding to protect the operator and the other employees from the 2'' opening on the rotating shaft of the new portable chemical transfer pump from the blender to the mud tanks. By not guarding the rotating parts on the chemical transfer pump, employees were exposed to laceration hazards.    (c) Cyclone Drilling, Inc., Drilling Rig 17, API 05-045-19478, Rifle, CO 81650: On and before July 7, 2011 the creating, exposing and correcting employer did not provide one or more methods of guarding to protect the operator and the other employees from ingoing nip points on the orange pneumatic, air winch, Model XJFH-5/35.  This winch is positioned at the top of the steps leading to the rig floor and is approximately 4 feet off the rig floor.  This condition exposed employees to caught-in hazards.
