Tiffin, OH: The employer did not ensure employees were protected from head injuries while working below employees conducting roofing work above. Employees were exposed to potential impact injuries caused from falling material and/or equipment.

29 CFR 1926.451(e)(1): Scaffold platforms more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used. (a) Charles Ranker & Sons, Inc. on or about July 14, 2011, at the address of 10 Wellington Pl. Tiffin, OH: The employer failed to ensure a ladder or similar safe means of access was provided to employees using a welded frame scaffold. Employees were exposed to an 11ft fall to the ground below when utilizing a roofing material hoist to access the welded frame scaffold.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level was not protected from falling to that lower level. (a) Charles Ranker & Sons, Inc. on or about July 14, 2011, at the address of 10 Wellington Pl. Tiffin, OH: The employer did not ensure employees were protected from a fall while working from a welded frame scaffold at the Southeast corner of the structure. Employees were exposed to a near 11 ft. fall to the ground below while utilizing the scaffold both as a material staging area as well as an access to the roof level. (b) Charles Ranker & Sons, Inc. on or about July 13, 2011, at the address of 10 Wellington Pl. Tiffin, OH: The employer did not ensure employees were protected from a fall while working from a scaffolding pick supported by two sections of welded frame scaffold at the East side of the structure. Employees were exposed to a near 16 ft. fall to the ground below while working from the scaffold to fasten the first few rows of shingles prior to installing roof jacks.
