29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary:    a)  On or about July 13, 2011, employees were exposed to injury while using an Alum-A-Pole pump jack scaffold where the intermediate braces had been removed and work was still permitted from the pump jack.  The scaffold poles were installed at approximately 30 ft in height.      Abatement Note: The manufacturer requires additional bracing when the height exceeds 24 ft at intervals no greater than 16 ft.    Abatement Note: Abatement certification is required for this item.
