OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion/fire hazards. a) On or about June 29, 2011 and at times prior, Halliburton Energy Services did not ensure that employees were protected from explosion/fire hazards. 1. Signage warning of electrical device usage at the entry to well site was not maintained or displayed. 2. The proper number and size of fire extinguishers was not maintained at explosives transport truck. 3. The bed of the truck used to transport the explosives was not spark proof. 4. No straps or other method was used to secure the explosives in the truck bed. 5. Flammable items (cans of spray paint) were stored and transported with the igniters and explosives. Among other methods one feasible and acceptable abatement method is to follow the standards of the industry as llisted by Colorado State Division of Oil and Public Safety, Explosive Regulations Chapter V.

Employees removing and replacing sections of flexible piping, from the top of the jell hydro tank, were exposed to falls of over 10 feet.
