OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion/fire hazards. a) On or about June 29, 2011 and at times prior, The Perforators LLC did not ensure that employees were protected from explosion/fire hazards. 1. Signage warning of electrical device usage at the entry to well site was not maintained or displayed. There were no signs warning of explosives at the gun assembly area. 2. The proper number and size of inspected and tested fire extinguishers was not maintained at the explosives transport truck. 3. The bed of the truck used to transport the explosives was not spark proof. 4. No straps or other method was used to secure the explosives in the truck bed. 5. Flammable items (cans of spray paint) were stored and transported with the igniters and explosives. 6. The guns were being armed just outside the control trailer. This area was not free of electrical devices, stray current, etc. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
