29 CFR 1926.451(g)(1)(i):     Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:    Instance 1. On July 13, 2011 an employee was not provided fall protection while working from a ladder jack scaffold on the north side of the building, exposing him to a fall hazard of 20 feet.    Instance 2. On July 13, 2011 an employee was not provided fall protection while working from a ladder jack scaffold on the south side of the building, exposing him to a fall hazard of 20.3 feet.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.    On July 13, 2011 employees were not provided a safe means of access to the ladder jack scaffold they were working from, exposing them to the hazards associated with no safe access.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest system.      Instance 1. On July 13, 2011 employees were not provided fall protection while roofing a commercial like building with a flat roof, exposing them to a fall hazard of 25 feet.      Instance 2. On July 13, 2011 employees were not provided fall protection while roofing a commercial like building; the employee was exposed to a 17 foot fall as he worked inside a window well on the second floor.    Abatement documentation is required of this item in accordance with 29 CFR 1903.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about July 13, 2011 employees were not trained to recognize or minimize the hazards associated with working at heights, exposing them to fall hazards between 17 and 25 feet above grade.    Abatement certification is required of this item in accordance with 29 CFR 1903
