On or about June 8, 2011 while working below employees tearing off a roof, the employer did not ensure hard hats were worn to protect employees from being struck in the head.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: On or about June 8, 2011, while performing tear off and replacement of weatherproofing from a residential roof, the employer failed to ensure employees working from a 10/12 pitch main roof were protected from a fall of approximately 17 feet 10 inches by using an adequate fall protective system/devices.
