Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fracture and amputation hazards:\\\\r\\\\n\\\\r\\\\n(a) Cable Room - Provision was not made to prevent unintended operation (actuation) onto the foot pedal on the AMP pneumatic press from falling or moving objects or by accidently stepping.\\\\r\\\\n\\\\r\\\\nAmong other methods, one feasible and acceptable way to correct this hazard includes, but not limited to the following:\\\\r\\\\n\\\\r\\\\nConform to the American National Standards Institute ANSI B11.3-2002, Sections 6.10.2 and 6.10.3, which states that \\\\\\\"When provided, foot controls shall be protected to prevent actuation from falling or moving objects or from unintended actuation by providing a foot pedal and treadle guards.\\\\r\\\\n\\\\r\\\\n

29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee\\\'s responsibilities under the energy control procedure being inspected.\\r\\n\\r\\n(a) Facility Wide - The employer did not conducted annual periodic inspections of the energy control procedures for each authorized employee.\\r\\n\\r\\nIn accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:\\\\r\\\\n\\\\r\\\\n(a) Machine Shop - The Hardinge-HLV-H lathe was not equipped with enclosure guards for their rotating chucks and chip guards to contain flying chips or sparks.\\\\r\\\\n\\\\r\\\\n(b) Cable Department - The cable pneumatic press was not properly guarded to prevent employees from entering the point of operation.
