29 CFR 1910.147(c)(6)(i): \r\n\r\nThe employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:\r\n\r\n\r\na) Entire facility- On or about 07/14/11, the employer did not conduct an annual or more frequent inspection of the energy control (lockout/tagout) procedures to ensure that the procedure and the requirements of this standard were being followed.\r\n\r\nBradrock Molding Company, Inc dba Bradrock Industries was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard which was contained in OSHA Inspection Number 312110059 Citation Number 1, Item Number 2, issued on 06/04/08, with regard to the workplace located at 75 East Bradrock Drive, Des Plaines, Illinois.  The final order date was June 23, 2008.\r\n\r\nIn accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.\r\n\r\n
