The openings in the point of operation guard were too large and did not prevent employees' hands from entering the point of operation of the shear blade while cutting various size pieces of sheet metal.    Abatement documentation and certification is not required for this instance.  Corrected During Inspection.    b)  Uni-Hydro 80 Ton Iron Worker Machine, foot pedal operation.  The point of operation was not guarded at the punching station to prevent employees' hands from entering the point of operation while punching holes in small pieces of sheet metal.    Abatement documentation is required for this instance in accordance with 29 CFR 1903.19(d).
