29 CFR 1926.20(b)(2): The employer did not initiate programs that provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.\r\n\r\n\r\n\r\n\r\n(a) Job Site: On or about June 28, 2011, and at times prior thereto, the controlling contractor, Lifetime Roofing & Siding, did not initiate through inspections to ensure that its sub-contracted employer Farman Roofing, LLC protected its employees while they were exposed to hazardous conditions such as but not limited to fall hazards, overhead hazards, potential eye injuries, and not utilizing appropriate personal protective equipment to minimize the hazards.\r\n\r\n\r\n\r\n\r\nNote: Abatement certification IS required for this item.

29 CFR 1926.501(b)(11): \"Steep roofs.\" Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.\r\n\r\n(a) Wallace Library: On or about June 28, 2011, the controlling contractor, Lifetime Roofing & Siding did not ensure that its sub-contracted employees involved in roofing activities on a 7 in 12 pitch with a 27 foot ground to eave height were adequately protected from a potential fall hazard.\r\n\r\n\r\n\r\nNote: Abatement certification IS required for this item.
