29 CFR 1926.501(b)(10):     "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safetymonitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted.    R. Callahan Construction Company had employees exposed to greater than 30 ft. fall hazards without fall protection, on a commercial construction site in Sullivan, Illinois.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed inorder to minimize these hazards.\r\n\r\nR. Callahan Construction Company had employees exposed to greater than 30 ft. fall hazards without fall protection, on a commercial construction site in Sullivan, Illinois.  The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards of falling and the safety requirements associated with fall protection, as well as the proper work procedures to follow, in order to minimize and eliminate the fall hazards.

29 CFR 1926.1053(b)(1):     When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder.  In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.    R. Callahan Construction Company had employees exposed to 12 ft. fall hazards while exiting and accessing the roof via an extension ladder that did not extend to the upper landing surface of the roof, on a commercial construction site in Sullivan, Illinois.

The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.\r\n\r\nR. Callahan Construction Company had employees exposed to 12 ft. fall hazards while exiting and accessing the roof via an extension ladder that did not extend to the upper landing surface of the roof, on a commercial construction site in Sullivan, Illinois.   The employer did not provide construction safety training to employees, in order to provide them with the knowledge necessary to recognize the hazards related to ladders, as well as the proper work procedures to follow, in order to minimize and eliminate theses hazards.  This training would include:\r\n\r\na.\tThe nature of fall hazards in the work area, \r\nb.\tThe correct procedures for erecting, maintaining, and disassembling the fall protection systems to be used,\r\nc.\tThe proper construction use, placement, and care in handling of all ladders,\r\nd.\tThe maximum intended load carrying capacities of ladders, and \r\ne.\tThe standards contained in this subpart, Subpart X \u0013 Stairways and Ladders.\r\n

29 CFR 1926.453(b)(2)(v):     A body belt shall be worn and a lanyard attached to the boom or basket when working from anaerial lift.Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable aspart of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under1926.502(e).    R. The employer had an employee working from a JLG at heights greater than 30 ft. without a body belt and lanyard attached to the boom or basket of the JLG.

The employer did not provide scaffold training.  This training would include:\r\n\r\na.\tNature of any electrical hazards, fall hazards, and falling object hazards in the work area,\r\nb.\tCorrect procedures for dealing with electrical hazards and for erecting, maintaining, and disassembling the fall protection systems and falling object protection systems used,\r\nc.\tProper use of the scaffold, and the proper handling of material on the scaffold, \r\nd.\tThe maximum intended load and the load-carrying capacities of the scaffold used,\r\ne.\tAny other pertinent requirements of this subpart, L \u0013 Scaffolds.\r\n
