29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: On or about July 18, 2011, United Renovations, as the controlling employer, did not ensure that Burt Loudenlager, a sub-contactor, protected their employees from falls in excess of 14 feet while working off three pump-jack scaffolds with guardrails missing in the middle of the scaffolds and at the end of the scaffolds.
