29 CFR 1910.1001(j)(2)(i):  The employer did not determine the presence, location, and quantity of asbestos-containing material (ACM) and/or presumed asbestos-containing material (PACM) at the work site.  The building owner, facility owner or employer did not exercise a due diligent attempt to inform employees and/or other employers about the presence and location of ACM and PACM:  (a)  Mercy Holly Park East at 5546 E 60th Avenue, Commerce City, CO 80022:  On July 15, 2011, the employer did not determine the presence, location, and quantity of ACM and/or PACM at the work site.  The employer did not exercise a due diligent attempt to inform employees and/or other employers about the presence and location of ACM and PACM.  Unit 241 in Building 46, built in 1967, contained PACM in that the ceiling of the dining room contained textured surfacing material.  Two employees removed part of the ceiling in response to a water leak in the unit.  The employer did not determine the presence, location, and quantity of asbestos within the unit.  This condition potentially exposed employees to a respiratory hazard associated with asbestos.  Abatement Note:  Abatement certification is required for this item (see enclosed Sample Abatement Certification Letter).
