Roof- Two employees working from the roof were exposed to a fall of 12 feet where their personal fall arrest equipment was attached to the same anchor point which was not rated for more than 5,000 pounds.

Roof- An employee working from the roof eave was exposed to a free fall of greater than six feet where their vertical life line was attached to an anchor point several feet away from them horizontally at the roof ridge.

Roger Trento dba Royal Roofing was previously cited for a violation of this occupational safety its equivalent standard, 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 313124091, citation number 1, item number 1, issued on 6/2/2009, with respect to a workplace located at 13 River Street, Billerica, MA 01821. The final order date for the citation was on 6/23/2009 upon signing an expedited informal settlement agreement.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a. Employees exposed to a fall of 12 feet while working from a roof were not provided with fall protection training from the employer. Two employees were not using fall protection equipment and three employees were using personal fall arrest equipment incorrectly.     Roger Trento dba Royal Roofing was previously cited for a violation of this occupational safety standard which was contained in OSHA inspection number 313124091, citation number 1, item number 2, issued on 6/2/2009, with respect to a workplace located at 13 River Street, Billerica, MA 01821. The final order date for the citation was on 6/23/2009 upon signing an expedited informal settlement agreement.
