Employees were exposed to electrical shock hazards when a piece of wire hanger was used as a means to open three breaker switches at once in the main high voltage electrical breaker panel outside of spray booth #2.

29 CFR 1910.107(i)(4): Electrical support equipment. Transformers, powerpacks, control apparatus, and all other electrical portions of the equipment, with the exception of the handgun itself and its connections to the power supply shall be located outside of the spraying area or shall otherwise conform to the requirements of paragraph (c) of this section. Electrical light switches and associated receptacles in the spray booths spraying black powder (the two booths to the left of the wash bay) did not conform to the Class II Division 2 electrical requirements, potentially exposing employees to combustible dust ignition hazards. Repeat: Kellerman Powder Coating, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard, 29 CFR 1910.107(i)(4), which was contained in OSHA Inspection Number 313015620. Citation 1 - Item 2, and was affirmed as a Final Order on 12/15/2010, with respect to a workplace located at 3543 U.S. Route 51, DuQuoin, IL 62832.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals intheir work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trainedabout is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability,carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety datasheets. Employees did not receive training on hazardous materials upon initial assignment or when new products were introduced in their work areas and were exposed to chemical hazards from POWDURA powder coat paint in that employees were not trained on the potential explosive nature of the product. Repeat: Kellerman Powder Coating, Inc. was previously cited for a violation of this Occupational Safety and Health Standard, or its equivalent standard, 29 CFR 1910.1200(h)(1), which was contained in OSHA Inspection Number 313015620. Citation 1 - Item 11, and was affirmed as a Final Order on 12/15/2010, with respect to a workplace located at 3543 U.S. Route 51, DuQuoin, IL 62832.
