29 CFR 1910.132(a): Application. Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.    In the Production Area employees were exposed to an approxmately 7' fall hazard when cleaning out the hopper located on top and to the rear of the #5 Plastic Injection Molding Machine.

29 CFR 1910.137(b)(2)(viii): Electrical protective equipment shall be subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-5 and Table I-6.    Insulated rubber gloves used by an qualified employee while working on energized electrical circuits had not been tested since their purchase, 4-5 years ago.

29 CFR 1910.335(a)(1)(i):Employees working in areas where there are potential electrical hazards shall be provided with,and shall use, electrical protective equipment that is appropriate for the specific parts of the body to be protected and for the work to be performed. Note: Personal protective equipment requirements are contained in subpart I of this part.    On or about August 2, 2011, in the Clean Room, an employee was voltage testing using a multimeter on an energized 200 Volt AC circuit on the nut changer for the End Form Machine and was not wearing insulated rubber gloves and FR cloithing to protect himself against electric shock and arc flash.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a.  In the Clean Room an employee was exposed to the robotic movement of the manual bending machine for the metal fuel line due to the lack of a safeguarding system inside the danger zone of the machine operating envelope.    b.  In the Clean Room an employee was exposed to the point of operation on the Bushing Machine due to the lack of a safeguarding system.

On or about August 1, 2011, in the Production Area, an employee inspecting and cleaning out the inside of the Performance Tester Machine during a "fly off" did not lock out the electirical and pneumatic energy sources. 29 CFR 1910.37(a)(3): Exit routes must be free and unobstructed. No materials or equipment may be placed, either permanently or temporarily, within the exit route. The exit access must not go through a room that can be locked, such as a bathroom, to reach an exit or exit discharge, nor may it lead into a dead-end corridor. Stairs or a ramp must be provided where the exit route is not substantially level.    In the Parts Recycling Area, the exit route and door were blocked by an airline running across the door, and boxes of parts laying on the floor in front of the exit route.
