29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:    (a) Smart Roofing, LLC at 2543 W 105th Ct, Westminster, CO 80234:  On and before June 28, 2011, the controlling employer did not ensure that subcontractor employees were protected from injury, in that four subcontractor employees removing shingles from a 4.5-in-12 pitch roof on a two-story residential home were not utilizing a fall protection system. Subcontractor employees worked along the eave of the roof on the 2nd level with their back towards the edge of the roof. This condition exposed subcontractor employees to a fall hazard of approximately 17.5 feet.
