29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a) Well Done Construction, Findlay, Ohio:  On or about July 15, 2011, at address 2416 Sweetwater Rd., Findlay, OH, the employer failed to ensure that each employee was protected from head injuries while working below employees conducting roofing work above.  Employees were exposed to potential impact injuries caused from falling materials and/or equipment while picking up scrap materials from the ground below the working level.

29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a) Well Done Construction, Findlay, Ohio:  On or about July 15, 2011, at address 2416 Sweetwater Rd., Findlay, OH, the employer failed to ensure that each employee was provided appropriate eye protection while engaged in nailing activities, such as securing shingles to the structure roof with a pneumatic nailer, exposing employees to eye injuries from flying particles.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor  did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    a) Well Done Construction, Findlay, Ohio:  On or about June 15, 2011, at address 2416 Sweetwater Rd., Findlay, OH, the employer did not ensure that employees were protected from falls while applying new roofing to a low sloped roof (4:12 pitch) on a residential type structure.  Employees were exposed to a potential fall of approximately 17.8 feet to the lower level below. \\r\\n

29 CFR 1926.1053(b)(4):  Ladder(s) were used for purposes other than the purpose for which they were designed:    a)  Well Done Construction, Findlay, Ohio:  On or about July 15, 2011, at address 2416 Sweetwater Rd., Findlay, OH, the employer failed to ensure that a 10 ft.  A-framed stepladder was opened during use.  The ladder was closed and leaning against the structure while employee used it to access the upper portion the roof.  Employees were exposed to fall hazards due to the improper placement and use of this ladder.
