29 CFR 1910.268(d)(1): Before work was begun in the vicinity of vehicular or pedestrian traffic which could endanger employees, warning signs, flags, barriers, and/or other traffic control devices were not placed conspicuously to alert and channel approaching traffic: a) GLW Broadband, Inc., on or about July 20, 2011 at the location of 18800 Whitehead, Lagrange, OH: The employer did not assure employees performing telecommunication line work from a mobile extensible boom platform were adequately protected from struck by hazards. The employees did not use any means to alert traffic while working on the roadway.

29 CFR 1910.268(e): Personal protective equipment, protective devices, and special tools needed for the work of employees were not provided and/or the employer did not ensure that they were used by employees: a) GLW Broadband, Inc., on or about July 20, 2011 at the location of 18800 Whitehead, Lagrange, OH: The employer did not assure employees performing telecommunication line work from a mobile extensible boom platform were adequately protected from falls. The employees operating the lift in this condition were exposed to a 20 foot fall to the ground below.
