29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a: Employees were exposed to falling 11ft through the unguarded floor openings to the concreted level below as they traveled the walking/working surfaces of the site. **In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.**

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems: a) Employees were exposed to fall hazards up to 30 feet while engaged in steel erection activities on the 3rd level of the single family dwelling at the northwest side of the site. b) Employees were exposed to fall hazards up to 20 feet while engaged in steel erection activities on the 3rd level of the single family dwelling at the East side of the site. Skyline Iron Works, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.760(a)(1), which was contained in OSHA inspection number 315180497, citation number 01, item number 001 and was affirmed as a final order on February 14, 2011, with respect to a workplace located at 3501 N. Clark in Chicago, Illinois 60657. **In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
