29 CFR 1926.21(b)(2):  The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury.      (a)	Christina Janway, dba C & M Construction, 100 Derrer Meadow, Columbus, Ohio:  On or about July 19, 2011, at the work site 410 Harcourt, Vermilion, Ohio, the employer did not ensure that an employee working near energized electrical lines was trained in the recognition, control, and elimination of the hazards associated with energized lines.  The employee was not aware of the hazards associated with working near energized electrical circuits and the means to protect from those hazards.

29 CFR 1926.95(a):  Personal  protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition, whenever it was necessary by reason of hazards of process or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact.    (a)	Christina Janway, dba C & M Construction, 100 Derrer Meadow, Columbus, Ohio:  On or about July 19, 2011, at the work site 410 Harcourt, Vermilion, Ohio, the employer did not ensure that an employee working near energized electrical lines was protected from a shock hazard.  While cleaning roofing debris from the gutters an employee received an electrical shock when contact with the exposed, energized electric lines occurred.

29 CFR 1926.416(a)(1):  The employer permitted an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.      (a)	Christina Janway, dba C & M Construction, 100 Derrer Meadow, Columbus, Ohio:  On or about July 19, 2011, at the work site 410 Harcourt, Vermilion, Ohio, the employer did not ensure that the electrical lines servicing the residence were deenergized or effectively insulated.  While cleaning roofing debris from the gutters an employee was shocked when contact with the exposed, energized electric lines occurred.

(a)	Christina Janway, dba C & M Construction, 100 Derrer Meadow, Columbus, Ohio:  On or about July 19, 2011, at the work site 410 Harcourt, Vermilion, Ohio, the employer did not ensure that an employee working near energized electrical lines used a ladder with nonconductive side rails.  While cleaning roofing debris from the gutters, an employee used an aluminum extension ladder to access the gutters.  The employee was shocked when contact with the exposed, energized electric lines occurred.
