OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to burn hazards from hot exhaust pipes from the Hydro Tek Mobile Wash Skids: (a) S&J Slater Enterprises, LLC, dba A Clean Slate @ 5810 Broadway, Denver, CO 80216: On and before 8/10/11, employees were exposed to hot exhaust pipes from the Hydro Tek Mobile Wash Skids. When checking on equipment inside the box van, employees could contact the uninsulated pipes. This condition exposed employees to thermal burns.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to carbon monoxide from the Hydro Tek Mobile Wash Skids: (a) S&J Slater Enterprises, LLC, dba A Clean Slate@ 5810 Broadway, Denver, CO 80216: On and before 8/10/11, employees were exposed to carbon monoxide in the box portion of the equipment truck. Employees access the box portion regularly to check on equipment where carbon monoxide levels were observed in excess of 50 parts per million inside the confined space. This condition exposed employees to a hazardous atmosphere.

This condition exposed the employee to noise induced hearing loss.
