Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to carbon monoxide from the Hydro Tek Mobile Wash Skids:    (a) S&J Slater Enterprises, LLC, dba A Clean Slate@ 5810 Broadway, Denver, CO 80216: On and before 8/10/11, employees were exposed to carbon monoxide in the box portion of the equipment truck. Employees access the box portion regularly to check on equipment where carbon monoxide levels were observed in excess of 50 parts per million inside the confined space. This condition exposed employees to a hazardous atmosphere.

Employees work with and around carbon monoxide producing equipment (i.e. gasoline and diesel fuel engines). This condition exposed employees to respiratory hazards.

Employees are exposed to carbon monoxide generated from gasoline compressors and diesel heaters that are used in their daily pressure spraying activities. This condition exposed employees to respiratory hazards.

This condition exposed the employee to noise induced hearing loss.

Employees conduct pressure spraying operations which require the use of noise producing equipment such as, but not limited to, gasoline-powered vacuum systems and gasoline and diesel fueled generators.
