29 CFR 1926.95(a):     Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:  (a)  At the Riverwood Condo jobsite; a 3' lanyard, used as part of a personal fall arrest system, had a knot reducing the capacity exposing employees to fall hazards.

29 CFR 1926.501(b)(13): \r\n\r\nEach employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.\r\n\r\n(a)  At the Riverwood Condos jobsite; fall protection was not utilized while performing roofing activities exposing an employee to a fall hazard of approximately 17.1'.

29 CFR 1926.502(d)(16)(iii): \r\n\r\nPersonal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:\r\n\r\n(a)  At the Riverwood Condo jobsite; the rope grabs were located at the end of the rope of the personal fall arrest system which could allow employees to hit the ground before stop falling.
