Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. On the site, the employer did not ensure the employee using a saw to dry cut concrete was utilizing eye protection, hearing protection and a dust mask.

29 CFR 1926.403(b)(1): Examination. The employer shall ensure that electrical equipment is free from recognized hazardsthat are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined on the basis of the followingconsiderations: a. On the site, the employer did not ensure the extension cord being used on the site to provide power to the circular saw was in good condition. The casing on the cord was torn and wires were exposed.
