29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, and enclosures was not permanent and continuous: a) ADB Construction, Inc. at 483 North Jackson Way, Aurora, CO 80018: On or before 7/23/11, the employer did not ensure the following equipment was continuously grounded: 1) The Skilsaw that was used to cut siding had a broken ground plug; and 2) The extension cord used to power the Skilsaw had an open ground. Condition exposed employees to electrical shock hazards. Abatement note: Abatement certification is required for this item (See enclosed "Sample Abatement-Certification Letter").
