29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts:     a)  On or about July 25, 2011, the Kingston HJ-100 horizontal metal lathe  located in the building #2 Maintenance Repair Department did not have a chuck guard installed to protect against accidental contact with the rotating chucks.        b)  On or about July 25, 2011, the South Bend 400 horizontal metal lathe located in Second Operations Department did not have a chuck guard installed to protect against accidental contact with the rotating chucks.          c)  On or about July 25, 2011, the HI Tec-Turn 20 SII CNC machine identified as number 20 located in building 2 on the main floor area did not have a working interlock door switch to stop the machine when the door was opened to protect the operator from accidental contact to rotating tooling equipment and parts.    d)  On or about July 25, 2011, the Oil Re-claimer equipment located outside the main machine shop and used to remove used oils from metal chips and shavings by means of centrifugal force did not have a lid that had a working latch and an interlock switch to protect the operator from rotating material and equipment during the equipment cycle.
