29 CFR 1926.451(f)(1): Scaffold and/or scaffold components were loaded in excess of their maximum intended loads or rated capacities, whichever was less: a) On or about July 26, 2011, employees were exposed to fall hazards when working on a scaffold that was loaded in excess of the maximum intended load. The overloading of the scaffold contributed to the platform collapsing.
