29 CFR 1926.100(a):     Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a)  Midwest Environmental Control, Inc., on or about July 28, 2011 at the site of 625 Greene St., Toledo, OH:  The employer did not ensure employees were protected from head injuries while removing asbestos.  The exposed employee was working below another employee who was removing cementitious asbestos-containing siding.   Hazards include falling tiles and possibly hand tools in use by the employee working above.

(a)  Midwest Environmental Control, Inc., on or about July 28, 2011 at the site of 625 Greene St., Toledo, OH:  The employer did not ensure employees were provided with proper eye protection while removing cementitious asbestos-containing siding using hand tools.   Hazards include pieces of asbestos tile and paint chips that become projectiles during the demolition process.

29 CFR 1904.29(b)(1):     A log of all recordable work-related injuries and illnesses (OSHA 300 or equivalent), was not completed in the detail as required by the regulation:    Midwest Environmental Control, Inc., Toledo, Ohio:  On or about July 28, 2011, the employer did not complete an OSHA 300 in the detail required by the regulation.     a)  Injury and illness records provided by the employer were reviewed and indicated that on or about March 1, 2010, a recordable injury occurred when an employee was treated for a rash from chemical exposure.  The employee received medical treatment as a result. The employer did not ensure that the part of the body affected was recorded in column F of OSHA log; columns A and E were left blank; and a category under G, H, I, or J was not marked.    b)  Injury and illness records provided by the employer were reviewed and indicated that on or about January 8, 2010, a recordable injury occurred when an employee suffered a head injury.  The employer did not ensure that a description of the injury and the substance or object that caused it were recorded in column F of OSHA log; columns A and E were left blank, and a category under G, H,  I, or J was not marked.

The employee had restricted activity for 4 days. The employer did not ensure that the object or substance that directly caused the injury was recorded in column F of OSHA log; column A was left blank and E did not contain the location where the event occurred.    c)  The injury and illness records provided by the employer were reviewed and indicated that on or about June 11, 2009, a recordable injury occurred when an employee had a arm injury.  The employee was restricted for 30 days. The employer did not ensure that a description of the injury and the substance or object that caused it were recorded in column F of OSHA log.; column A was left blank and E did not contain the location where the event occurred.     d)  The injury and illness records provided by the employer were reviewed and indicated that on or about August 31, 2009, a recordable injury occurred when an employee cut their finger.  The employee the received medical treatment as a result of their injury. The employer did not ensure that the object or substance that directly caused the injury was recorded in column F of OSHA log; column A was left blank and E did not contain the location where the event occurred.    e)  The injury and illness records provided by the employer were reviewed and indicated that on or about October 2, 2009, a recordable injury occurred when an employee suffered a shoulder sprain.  The employee had restricted activity for 3 days.  The employer did not ensure that the object or substance that directly caused the injury was recorded in column F of OSHA log; columns A was left blank and E did not contain the location where the event occurred.    f)  The injury and illness records provided by the employer were reviewed and indicated that on or about December 8, 2009, a recordable injury occurred when an employee had a knee injury.  The employee was restricted for 17 days.  The employer did not ensure that a description of the injury and the substance or object that caused it were recorded in column F of OSHA log; columns A was left blank and E did not contain the location where the event occurred.
