29 CFR 1926.405(a)(2)(ii)(I): \r\n\r\nFlexible cords and cables were not protected from damage. Sharp corners and projections were not avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage.\r\n\r\nOn 7/7/11 an extension cord was run from the ground to the roof without protection from being cut by the edge of the sheet metal roofing.

The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under1926.502(e).\r\n\r\nOn 7/7/11 an employee operating a boom lift was not wearing a harness and lanyard.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section,each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall beprotected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrailsystem, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safetymonitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring systemalone [i.e. without the warning line system] is permitted. On 7/7/11 employees of Walker Meral Building Erectors, LLC were working at or near the edge of the roof without adequate fall protection in place. Walker Metal Building Erectors, LLC was previously cited for a violation of this Occupational Health and Safety Standard or its equivalent Standard, which was contained in OSHA Inspection Number 309838506, Citation #1, Item #1, and was affirmed as a Final Order on 7/23/07, with respect to a workplace located at 3600 Evergreen Dr., Appleton, WI 54913
