On or about July 25, 2011, at 1350 Moore Road in Avon, Ohio, the employer did not ensure employees were protected from falls hazards while loading a semi-trailer. The forklift operator was still loading the semi-trailer at Dock #1 when the operator of the truck pulled the semi-trailer away from the dock which resulted in the forklift and the powered industrial truck operator becoming wedged between the semi-trailer and the loading dock.
