OSH ACT of 1970 Section (5)(a)(1):     The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall, crush, and struck by hazards while loading and unloading semi-trailers.    a.) On or about July 25, 2011, at 1350 Moore Road in Avon, Ohio, the employer did not ensure employees were protected from falls hazards while loading a semi-trailer. The forklift operator was still loading the semi-trailer at Dock #1 when the operator of the truck pulled the semi-trailer away from the dock which resulted in the forklift and the powered industrial truck operator becoming wedged between the semi-trailer and the loading dock.

Throughout the entire OSHA 300 Log, the descriptions in Column F lacked the required information, including, but not limited to the part of the body affected in the following instances:  1.) CASE NO: 09-013009- On January 30, 2009 an employee slipped and fell on some ice in Oateys parking lot. The entry does not specify the part of the body affected.  2.) CASE NO: 09-20909  On February 9, 2009 an employee slipped on ice while removing snow from his trailer. The entry does not specify the part of the body affected.  3.) CASE NO: 09-32709  On March 27, 2009 an employee fell going up the stairs at cosignee. The entry does not specify the part of the body affected.
