29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them):    GC Builders LLC, at 4672 W 20th Street Road, Greeley, CO: On 7/29/11, GC Builders LLC did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person.  GC Builders LLC was constructing a wood frame two story apartment building.  Workers installed windows on the first and second floors.  GC Builders LLC did not perform an inspection of the worksite and equipment to ensure that workers were not exposed to a hazardous work condition.  This condition exposed workers to a fall hazard and to a flying object hazard.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure:    GC Builders LLC, at 4672 W 20th Street Road, Greeley, CO: On 7/29/11, GC Builders LLC  did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, personal fall arrest systems, or another provision in paragraph (b) of this section which provides for an alternative fall protection measure.  GC Builders LLC was constructing a wood frame two story apartment building.  Workers installed windows on the first and second floors.  One employee had installed approximately five windows from the exterior while standing upon the first story roof.  Approximate height was twelve feet above ground level.  The employee was not protected by a guardrail system, safety net system, or personal fall arrest system.  This condition exposed one employee to an approximate twelve foot fall hazard.

One employee had installed approximately five windows from the exterior while standing upon the first story roof.  Approximate height was twelve feet above ground level.  GC Builders LLC had not provided fall protection training to its employee.  This condition exposed one employee to an approximate twelve foot fall hazard.

GC Builders LLC was constructing a wood frame two story apartment building.  Workers installed windows on the first and second floors.  Workers used hammers and nails to install the windows.  Workers did not use eye protection.  This condition potentially exposed workers to a flying object hazard.
