29 CFR 1910.305(b)(1)(ii):   Unused openings in cabinets, boxes, and fittings shall be effectively closed.  a) At the Margaret's Cafe located in Toronto, Ohio the employer had a 4 inch electrical box that housed the switches for the ventilation system above the stove and fryers that had a missing knockout.

29 CFR 1910.305(b)(2)(i):   All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) At the Margaret's Cafe in Toronto, Ohio there was an outlet receptacle located in the kitchen, underneath the dishwasher that was missing a cover.

29 CFR 1910.305(g)(1)(iv)(A): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:\\n\\nAs a substitute for the fixed wiring of a structure;\n\na) At the Margaret\\\'s Cafe facility located in Toronto, Ohio, the employer had used an extension cord to bring power to the the cooler located next to the automatic dishwasher.\n\nb) At the Margaret\'s Cafe facility located in Toronto, Ohio, the employer had used multiple extension cords to bring power from the prep station over to the dishwasher's station in order to power a radio and three fans.

29 CFR 1910.157(e)(3): \\n\\nThe employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request.\\n\\na) At the Margret\\\'s Cafe facility, the employer had fire extinguishers in the building that had not had an annual maintenance check since 2009.
