29 CFR 1910.147(c)(1):     The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      The employer's lockout program did not include the setting up tasks on the following machinery:        a. Medium Line,          1. Hartffrd Threader, R-15, the guard was not interlocked         2. Robertson Hartford Threader, R-4, the guard was not interlocked       b. Large Line,         3. Asahisunic Cold Header, H20, the guard was not interlocked and the machine was not locked out during setup         4. Nedschroeff, BV531, the guard was not interlocked and the machine was not locked out during setup            c. Trimmer Area         5. Warren Trimmer, FAC-2, the guard was not interlocked         6. Waterbury Trimmer, FAC-3, the guard was not interlocked       d.  Batch & Queue Area         7. National Cold Header, H08, the guard was not interlocked         8. National Cold Header, H05, the guard was not interlocked    One feasible method of abatement would be to install a positive brake interlock on the guards.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.219(d)(1):     Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):    In the tool room, the belt and pulley on the Sunnen Home was not adequately guarded as the operator could reach the ingoing nip point around the guard.    Abatement certification is required for this item in accordance with 29 CFR 1903.19(c)

29 CFR 1910.335(a)(1)(i):     Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed:    Employees performing voltage testing, on voltages up to 480 volt, were not provided with, nor wore, appropriate personal protective equipment, such as, but not limited to:  fire retardant clothing, face shield, rubber gloves, etc.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
